Every Complaint or other document initiating a civil action shall be accompanied by a completed civil cover sheet. See form available on the Court’s website ( www.flsd.uscourts.gov). This requirement is solely for administrative purposes, and matters appearing only on the civil cover sheet have no legal effect in the action. If counsel becomes aware of an error in the civil cover sheet, counsel shall file and serve a notice that identifies the error. Effective December 1, 1994.
