PROBATE COURT OF GALVESTON COUNTY, TEXAS ADMINISTRATION ORDER 2022-9 IT IS ORDERED that effective October 1, 2009, all applications for probate of a will as a muniment of title where the decedent was over the age of 55 and died after March 1, 2005, will need to have filed before the court hearing the form Authorization for MERP Certification completed by the Medicaid Estate Recovery Program. … IT IS FURTHER ORDERED that applicants may white out the first two sections of the decedent's social security number after receipt of the completed form from the Medicaid Estate Recovery Program and before filing with the County Clerk's office. … IT IS ALSO ORDERED that his order shall replace Administrative Order 2009-02 as a revised MERP form has been created.
