One-page scan with heavy speckle; read by eye on the page and on two zooms and typed from the page (the speckle defeats machine OCR on this page, so no OCR cross-check was possible). The number '21MICR04' is handwritten at the head of the page. Printed as on the instrument: '123 Judicial District Court' in the signature block, '273rd District Court' beside '123rd Judicial District Court' in the two pending-case paragraphs.
