Verbatim from the operative section, harvested 2026-08-02; whitespace normalised only. • Upon the filing any civil or family proceeding, all parties not being represented by an attorney, must complete a Notice of Permanent Mailing Address, a copy of which is attached hereto, and a copy of which shall be served upon the respondent/defendant along with the summons and complaint/petition. • At the time of filing an answer, any party, not represented by an attorney, must affix to the answer their current permanent mailing address. • Within one (1) week of moving, any unrepresented party must complete a new Notice of Permanent Mailing Address and file the original with the Clerk of the Circuit Court in and for Santa Rosa County, Florida and forward a copy to all other parties noting thereon by a certificate of service the manner and date of delivery of said copy. • In those cases where a party was originally represented by an attorney and the attorney withdraws from further representation of the party, that party must then complete a Notice of Permanent Mailing Address within one (1) week from date of the attorney’s withdrawal of representation in the case. • The Clerk of the Circuit Court shall affix the Notice of Permanent Mailing Address to the inside file jacket opposite from all other pleadings with the most current Notice of Permanent Mailing Address clearly visible and unobstructed by other court filings. • In the event one file has multiple volumes, the most current Notice of Permanent Mailing Address shall be affixed to the latest volume in which court pleadings are being filed.
